Please read this important information carefully.

    General terms and conditions
Precision Health Ltd is providing a health screening service to you (and your employer).

Health screening is a snapshot of a limited range of health measurements on a single occasion. It will be interpreted by our doctors based only on that information. It is not a substitute for an ongoing relationship between you, your GP and/or your specialist healthcare providers.

Precision Health recommend that you take the results from this health screening to your GP/Specialist and have him/her integrate the results with your previous health information before making any lifestyle or other changes to your treatments.

Precision Health's personnel, equipment and procedures are certified as competent and accurate, but no-one is perfect. Every test has an inherent margin of error. Precision Health will generally recommend repeating abnormal health screening tests on a different day and in a different setting (with your GP/Specialist) before making significant changes to your healthcare regimen.

Sometimes, the unexpected happens. Precision Health will do its best to ensure your appointment goes ahead at the time and date planned but cannot be held responsible for external factors beyond our control that prevent this eg. power cuts, extreme weather, civil disturbance etc. Precision Health cannot be held responsible for any delay in diagnosis of any health conditions that may arise out of such a event.

How will Precision Health process my information?
We take your privacy very seriously and ensure that data transmission and storage is as secure as possible.

Your personal data and health data are transmitted using 128bit SSL encryption and stored in an encrypted database online and with an encrypted backup locally.
Your data will be stored in the EU by Jotform Ltd. Jotform do not have access to your data, because it is stored with 256-bit AES encryption to which only Precision Health have the key. Precision Health has a formal agreement with Jotform Ltd to process your data in accordance with EU data privacy laws.
Your name, email and mobile phone number will be shared with Esendex Ltd, in order to send you an access code by text message for your screening report. Esendex will store your name and mobile in accordance with  UK-GDPR Data Privacy regulations for the sole purpose of sending the text message. Your details will not be shared by Esendex with any 3rd party. The data will be stored for 2 years and then deleted.
Your screening report will be sent as an encrypted file to your email address.
We will hold your report in an encrypted local database with encrypted online backup for 2 years, after which we will erase it. Your data is, at all times, accessible only to authorised Precision Health personnel and will be used solely for the purpose(s) to which you have consented.

Precision Health will not share personally-identifiable data with any 3rd party without your express consent except where there is A. A Legal requirement to do so or B. An ethical requirement as set by the Medical Council of Ireland, to prevent life-threatening harm to you or to another person. In these exceptional circumstances, your consent will be requested. If it cannot be obtained, only the minimum amount of data required to fulfil our obligations will be passed on.
Despite all of these security measures, no-one can guarantee 100% security. If you do not agree to Precision Health using your data in this way, you should not precede with the screening appointment. Any of the tests planned to be done today can be done by your own GP.

At the end of this screening project, Precision Health will share anonymised, aggregated data with your employer for health planning and promotion purposes. You will not be personally identifiable in this data.

Precision Health Ltd is registered with the Data Protection Commissioner of Ireland as a Data Controller.

  • Please complete the checkboxes below*
  • If you have questions about this consent, you can email us on support@precisionhealth.ie or ask our clinician.

  • Sex (at birth)*
  • How long have you been working with Sisk?*
  • How do you think your health has changed over the last 12 months?*
  • How often do you have an alcoholic drink?*
  • How many units of alcohol do you drink on a typical day when you are drinking (1 unit is half a pint, small glass of wine or 1 measure of spirits)*
  • How often do you have six or more drinks on one occasion?*
  • Do you smoke?*
  • Do you use e-cigarettes (vaping)?*
  • Have you tried to quit?*
  • Would you be willing to participate in a smoking cessation programme?*
  • How often do you eat 5 or more portions of fruit or vegetables (at least 3 being vegetables)?*
  • How often do you drink full sugar drinks eg Coke, 7-Up, Red Bull, Monster?*
  • How often do you eat red meat?*
  • How often do you eat processed meats (luncheon meat, hot dogs etc.)?*
  • How often do you add salt to your food?*
  • The recommended daily intake of water is 1.6 Litres (avg female) to 2.0 Litres (avg male) (3-4 pints or 8-10 average glasses).*
  • Do you pay attention to nutritional information on food labels?*
  • Do you have any of the following (tick all that apply)
  • The Bowel Cancer Screening test looks for the presence of blood in your faeces (stool). Blood in the stool is abnormal. It may indicate cancer or bowel inflammation or haemorrhoids and should be followed-up with a visit to your GP and sometimes a colonoscopy (camera test into the bowel). The test involves collecting a sample of your stool from the toilet and posting the pre-prepared container to our lab partner.

Our screening nurse will discuss your risks again on the day of the screen and offer you a bowel screening kit. This is an optional test.

  • Do you regularly examine your testicles for lumps?*
  • Do you have any lumps in your testicles that have not been investigated yet by a doctor?*
  • Testicular Cancer is more common in men aged under 45 years old. Typically, one testicle will become larger than the other, or an irregular lump may be felt within the affected testicle. If detected early, testicular cancer can be effectively treated and cured.

Examining your testicles is easiest done in the shower. Hold a testicle in each hand and compare the size of the left and the right. Then run your fingers around the front and sides of each testicle - it should be smooth, like an egg. Lumps and tubes at the back of the testicle are normal.

If you are worried about a change in size or a lump in your testicle, go to your GP for an examination. Your GP may request an ultrasound scan of your testicle. This is quick and painless and may save your life.

  • Has your father or brother had prostate cancer before the age of 60?*
  • Do you have symptoms of bladder obstruction?
  • You have 1 or more risk factors for Prostate Cancer. This does not mean that you HAVE prostate cancer but it may be worthwhile monitoring this risk with a blood test called Prostate Specific Antigen. Raised levels of PSA (adjusted for age) can indicate the need for further investigation. There are other conditions that cause increased PSA, so sometimes these extra investigations determine that there is no evidence of prostate cancer. The extra investigations for a raised PSA test result are

    • Physical Examination by your GP, including a rectal examination
    • MRI scan of your prostate
    • Referral to a Specialist Urologist for a biopsy of your prostate gland

These tests obviously result in some discomfort, expense and worry. Some men would prefer to speak to their GP first or seek a referral to a Specialist Urologist to discuss the risks in greater detail. Other men would prefer to get all the tests done early and then discuss the results and their risk level with their GP or Urologist.

You can read more about the PSA test here

The nurse will ask you again about your prostate cancer risk on the day of the screening.

If you would like to have a PSA blood test done:

    • You should not ejaculate for at least 48 hours before the test
    • You should not exercise heavily for 48 hours before the test
    • You cannot have the PSA test if you have a urinary tract infection

(you might get a falsely-elevated result and need further testing)

  • Do you examine your breasts*
  • Do you feel any lumps in your breasts now?*
  • Are you enrolled in the BreastCheck screening programme?*
  • Breast cancer is more frequent in women over the age of 50 years and uncommon for women under 30 years. Although some women with a family history are at increased risk, most breast cancers are not genetically influenced.

The biggest non-genetic risks for breast cancer are:

    • Increasing age
    • Obesity
    • Alcohol

Self-examination can lead to earlier detection of breast lumps and increase the survival from breast cancer. It is important to get into the habit of examining your breasts every month and contacting your GP if you are concerned about any changes.

Your GP will examine your breasts and may request imaging (x-ray or ultrasound) and a biopsy. These tests can be done at a specialist breast clinic in most parts of the country.

For women over 50 years, regular x-ray screening (mammogram) may detect cancer earlier. The BreastCheck Programme is available free of charge via the HSE.

  • Are you up to date on your cervical smear tests?*
  • Have you had the cervical cancer vaccine?*
  • Cervical cancer risk is increased by infection with the Human Papilloma Virus (HPV) and smoking. The cancer can be detected at an early stage by regular screening tests (PAP Smears) which are recommended approximately every 3 years for women over 25 years of age.

If you are over 25 and have not had a smear in the last 3 eyars, you can check your eligibility and find a screening clinic by visiting CervicalCheck's website.

Women under 25 can get a vaccine to reduce the risk of HPV infection. This is usually given at school from 12 years old but can be requested via your GP.

  • In the past week, how many days did you do 30 minutes or more of moderate activity (e.g. walking, cycling, running)?*
  • Do you engage in resistance training (e.g. bodyweight, barbell, kettlebells etc.) 2-3 times a week?*
  • Do you do any muscle stretching or lengthening work (e.g. yoga, pilates, foam rolling)*
  • Work has a negative efect on my mental health*
  • Work does not impact my life outside of working hours*
  • I am aware of the mental health supports, resources and training that Sisk has to offer.*
  • Do you feel that Sisk supports your mental health*
  • Please wear loose-fitting clothing to your health screening appointment, this will facilitate taking the blood samples and ECG test.

You do not need to fast but you should avoid very rich or sugary meals on the day of the screening. You should avoid excessive amounts of alcohol in the 48 hours before the screening.

If you have any questions or concerns about the screening tests, please contact our support team on support@precisionhealth.ie.

If your question is a clinical one, the support team will put you in touch with our clinical team.
