Health Screening CONSENT FORM

General terms and conditions
Precision Health Ltd is providing a health screening service to you (and your employer).
Health screening is a snapshot of a limited range of health measurements on a single occasion. It will be interpreted by our doctors based only on that information. It is not a substitute for an ongoing relationship between you, your GP and/or your specialist healthcare providers.
Precision Health recommend that you take the results from this health screening to your GP/Specialist and have him/her integrate the results with your previous health information before making any lifestyle or other changes to your treatments.
Precision Health's personnel, equipment and procedures are certified as competent and accurate, but no-one is perfect. Every test has an inherent margin of error. Precision Health will generally recommend repeating abnormal health screening tests on a different day and in a different setting (with your GP/Specialist) before making significant changes to your healthcare regimen.
Sometimes, the unexpected happens. Precision Health will do its best to ensure your appointment goes ahead at the time and date planned but cannot be held responsible for external factors beyond our control that prevent this eg. power cuts, extreme weather, civil disturbance etc. Precision Health cannot be held responsible for any delay in diagnosis of any health conditions that may arise out of such a event.
How will Precision Health process my information?
We take your privacy very seriously and ensure that data transmission and storage is as secure as possible.
Your personal data and health data are transmitted using 128bit SSL encryption and stored in an encrypted database online and with an encrypted backup locally.
Your data will be stored in the EU by Jotform Ltd. Jotform do not have access to your data, because it is stored with 256-bit AES encryption to which only Precision Health have the key. Precision Health has a formal agreement with Jotform Ltd to process your data in accordance with EU data privacy laws.
Your name, email and mobile phone number will be shared with Esendex Ltd, in order to send you an access code by text message for your screening report. Esendex will store your name and mobile is accordance with UK-GDPR Data Privacy regulations for the sole purpose of sending the text message. Your details will not be shared by Esendex with any 3rd party. The data will be stored for 2 years and then deleted.
Your screening report will be sent as an encrypted file to your email address.
We will hold your report in an encrypted local database with encrypted online backup for 2 years, after which we will erase it. Your data is, at all times, accessible only to authorised Precision Health personnel and will be used solely for the purpose(s) to which you have consented.
Precision Health will not share personally-identifiable data with any 3rd party without your express consent except where there is A. A Legal requirement to do so or B. An ethical requirement as set by the Medical Council of Ireland, to prevent life-threatening harm to you or to another person. In these exceptional circumstances, your consent will be requested. If it cannot be obtained, only the minimum amount of data required to fulfil our obligations will be passed on.
Despite all of these security measures, no-one can guarantee 100% security. If you do not agree to Precision Health using your data in this way, you should not precede with the screening appointment. Any of the tests planned to be done today can be done by your own GP.
At the end of this screening project, Precision Health will share anonymised, aggregated data with your employer for health planning and promotion purposes. You will not be personally identifiable in this data.
